Question Formats

Single Select Question - Multiple-choice question that allows respondents to select only one answer.

Multi-select Question - Multiple-choice question that allows respondents to select multiple answers.

Likert Scale Question - Multiple-choice question that asks respondents to rate each answer provided on a multi-point scale. Respondents are only allowed to check one box for each answer option provided. Likert scales can range in size from two to nine checkboxes

Open Ended Textbox- Respondents type their responses into an open but limited length text field provided. Responses can be delivered to the client as verbatim or coded into defined bins and presented in a typical data table format.

Open Ended Memo - Respondents type their responses into an open memo field provided. Responses can be delivered to the client as verbatim.

Combination Multi-select - Open Ended Textbox- Respondents have the ability to answer a given set of Multiple-choice questions or to type their responses into an open but limited length text field provided. Responses can be delivered to the client as verbatim or coded into defined bins and presented in a typical data table format.

NOTE: Images and complex HTML layouts can be incorporated into any of the question formats listed above. See Exhibit I for examples of each question format.

Answer Formats

Fixed - answers presented in the order designated by the client

Alphabetical - answers arranged alphabetically (useful for long brand lists)

Random - each answer set is randomized for each respondent

Random with anchor - fixed response appears at the end of each randomized answer set

Branching - respondents branch from one question to another based on the answer they choose to an earlier question

Terminate - respondents are eliminated from the survey based on the answer(s) they choose to a given question

One Image Per Answer - each answer has an image associated with it. Images can be displayed with or without a text descriptor
